Tips for New Year changes

I want to make lots of changes in the New Year, cliched I know, but I respond well to that sort of goal setting. I want to do lots of things at once but I need some sort of plan, journal, goal setting type thing. Can any one recommend any tips or books or something?

I'm thinking things like healthy eating, exercise, reading certain books, housework etc. I like plans and lists and things but in need some sort of way of doing it. I've been looking at those Franklin Covey planners which a friend at work has, but the are really expensive. I'm just wondering if anyone can recommend some other form of goal setting planner type thing.

Does this make sense?

Psychologies magazine has a journal with it for January issue - its only about £3-4 so not too heavy on the purse! I got it last year and remember it was quite good in helping set goals and keep on track with them... I would imagine there will be something in the magazine itself as well.
