adding middle name onto birth certificate

Hello, my little one is 10 months old and i really would like to add this middle name (linked to a family member) on the birth certificate, i did not do it at the registry stupidly because I was not 100% sure on wanting baby to have a middle name but now I am regretting it deeply and would really like baby to have it. anyone done this and what did it entail?
