There IS a market for custom dolls on the web, people provide a photo of a child they want to recreate- not necessarily one who has died by any means!- or just decide on the hair, skin and eye colour etc

I saw the Half Ton Mum show, and it does seem, however, that that lady really wanted to get her story out to as many folks as possible so that people at her weight would try to get some help.
