I cant seem to find the forms that i need to complete. Forms 1 & 2 i think that they are.
Ive been on the DOH website and cant locate them.
Also, can you make head or tail of it?
Im struggling, it seems absolutely huge and its going to take me weeks to get through it all. How are you doing with yours?

well i have a folder with sub sections - i've updated cv, added a few certificates gathered this year, popped in my pdp (thats over ayear old and nothing achieved on it!), written some of those - "what you want from this learning experience and what you got" forms. I've treated it like my portfolio really.

and then there's the 15 staff members that I need to do with them aswell as nvq, ldaf and a rather new PC based roster system and then they'll be ESR come jan.....

life in in nhs just gets better!!

yes but where did you get your forms.
we have been asked to complete forms 1&2 on the computer, not by hand and i just cant see them on the DOH website.

www.dh.gov.uk/en/Publicationsandstatistics/Publications/PublicationsPolicyAndGuidance/DH_4090843

scroll all the way to the bottom and click on the very last bit to download - the forms are there.
I'm on mat leave at the moment, but was part of the roll out team in our trust about 18 months ago - what a nightmare!!

Agree with twinklytoes, just use your portfolio/profile and pull out the relevant bits. The whole idea of KSF should be that you don't have to produce extra work (apart from the odd reflection or two which i guess we're 'supposed' to be doing anyway!) and all evidence can be found within your usual line of work.

hee hee, yes it does

Back in Feb though - can't say I'm looking forward to it though! Our trust has been hugely overspent and as a result of 'workforce change', my job no longer exists . So, I've got to go back to something that I don't really want to do - ah well, the joys of the nhs lol!

Oh enjoy every moment off with your lo.
awful about your job tho, will you role change in a big way?
