"Remember Me" poem, does anybody know who its by?

Please help!
I'm looking for the words to this poem, that was read out at my grandads funeral the other week. Unfortunately, was crying so hard, didn't get to take the whole thing in, but i know it was lovely. My Auntie read it, but she is now working out of the Malaysia & not easily contactable.
have tried to put "remember me" in the search engine, but can't seem to get anywhere without having to subscribe to the poets society! Oh & ended up on a Michael Landon tribute site!(Little House On The Prairie for those who don't know who he is.)

Thanks, edodgy. I'm a blubberin' mess too, now.

That was read at an extremely sad funeral I went to ( my brother) and I liked the feeling that he wasn't far away.

My grandad died 31st aug this year. Sometimes think i can hear his voice. Was very, very close to him & nana.
Get really down at the mo. I know it'll pass but so hard.
Grandad had to have an op to get rid of a hospital bug. He was 82. He knew he probably wouldn't make it. but when he gathered us together he said, " If I don't make it, i don't want you to be sad. I've had a good life, got a great family, & been round the world more than twice, & got no regrets" ( he was in the RAF) I know he wouldn't want me to be sad. But sometimes at home when it's quiet, i think i can here things, & say is that you Grandad? Then i feel stupid!

Clare, Im sorry for your loss. You shouldnt feel stupid, after my dad died I felt him hug me and since my mum died they both come to me in dreams, i've learnt to tell the difference between dreams I have about them and dreams where they come to me. I asked my mum to come to me in dreams and i truly believe she has. I believe when you can feel your grandad he is with you.
