Organising a Coach with insurance what to do when too many passengers

I have heard of an incident today when a school coach was organised to take a group of children out and there were too many to fit on the coach by one child so he (age 11) was asked to sit on one of the teacher's laps for the journey there and back.

What are the insurance and other implications of this as I am rather concerned about it?

That absolutely shouldn't have happened. They should have booked a large enough coach or one of the parents or teachers should have followed by car.

Apparently they asked permission from the parent but phrased it in such a way that she assumed they were driving and she gave verbal permission on that basis.
