Need some help decifering what this is asking for.
DH had to miss his flight due to a stomach bug/food poisoning/something? Anyways he was feeling terrible.
This is the header of a document for his GP to fill in and sign. However we can't figure out what information it's asking for 🤷🏻♀️
"Date of trip booking" is date he was supposed to travel or the date he booked the flight?
"Date of balance payment" ...... I'm at a loss here...... date he paid for the flight? Date they give us the money back?..... what weird words together
