Help wording a letter to credit companies for a vulnerable person

My dad has power of attorney for an eldely lady who is becoming more and more confused. She has always ordered lots of stuff from catalogues, usually through the post or by debit card over the phone but there have been several instances recently where she has run up debt and excessive charges but swore blind she hasn't.

What we think is happening is that she is being contacted or when she phones is being asked if she wants to take advantage of a special discount but she doesn;t realise that she is actually opening multiple credit accounts.

My dad has just paid off the latest balance but he wants to write to this particular catalogue where she keeps running up debts to tell them not to open any more credit accounts in her name.

How does he word this? (he has to send a copy of the POA to them).

Look on national debt line website they have template letters

The ones I found were about asking companies to write debt off. I need wording to explain why a company shouldn’t give credit to this lady, something along the lines of not having the capacity to understand.
