Lost matb1 form

Does anybody know or have this happen to them before?

I have to send my original matb1 form to both my employers, the first I sent it to said they sent it back but I think it's lost as it hasn't arrived in over 2 weeks.

I've read I can get a duplicate from the midwife but will this have to be sent to the first employer again as they have specific serial numbers on and both employers would need the same form?
Or can the first employer use the one they saw and the second use the duplicate?

Did your first set of employers photocopy it? Could they send you a copy of the photocopy to give to your second employers?

Thankyou for the suggestion. I'll look into that. They have taken a photocopy.
Both employers ask for the original and it's my main employer who is waiting for the form, they also keep it for some reason.

My midwife told me they have no right to the original copy and a photocopy is fine. In fact she told me to photocopy it straight away and only give them that. All they need are the numbers off it.

Hmm, interesting. They kept mine last time. I will investigate. Stupidly I didn't photocopy it as I was expecting it to be returned.
