any one help draft a letter to request hospital notes?

For some private reasons, DP and I would like to see my notes from a hospital we were treated by very early in my pregnancy at the EPAU.

I have got an address to write to in order to request my notes, but can any one help me draft the letter?

Please, I have no idea of format?

I have never drafted a letter as such but keep it simple. Give dates of when you were under their care and request your medical records. I don't think that you have to give a reason. You may have to go in to read them- my GP would not give me a copy.

I had a form to fill in when I requested mine (I'm still waiting for them over a year later but that's another thread!) which asked things like which department, dates from and to and that's it. You shouldn't have to give a reason as they're obliged to give them you under the DP Act. Yoy may have to pay a fee though. Mine was £50.

Depends on the size of the notes. I was asking for all of my maternity notes and the notes relating to my subsequent ERPC. Smaller notes could cost as little as a tenner.

As far as I am aware you have the right to view them free of charge, if you then decide you want a copy they may charge you AIMS The Association for Improvement In Maternity Services will help you with this, they are excellent
