Maternity Allowance Form - Employment History Help

I'm filling out my Maternity Allowance form but I don't have the dates I was employed from in the the two jobs I need to put in. My current one should be easy enough to find out but the one before that was a shitty employer who I don't want to have to contact and have such a high turnover they probably don't even remember me.
Do these dates need to be exact? How do I find them out without having to print and fill in another form to send off? (why can't you just do this online already!?) Can I just phone HMRC and ask?
I don't want any delays or problems in this form being processed.

Hi, yes to be honest your dates need to be pretty much spot on. Ask your current employer to confirm your start date. If you call HMRC, they can supply you with a copy of your employment dates from previous employment. If you dont have the dates on your form, or have incorrect dates it can delay things and it takes long enough as it is!
