PPI reclaim

I am trying to reclaim some PPI back but I do not remember whether I was sold it in a branch or on line. I've scrolled through my emails but they don't seem to go back that far. I'm struggling to complete the form in view of this. Does this matter? Surely we can't all remember these small details. Anyone have experience of this?

My reason for reclaiming is that if it had been explained to me properly I would not have needed it as my I would have had statutory sick pay and my husband could have helped with payments so it wasn't necessary for me.

It matters because if you were 'sold it' online then you choose to tick a box to include/buy it, rather than in a branch where someone will have sold it to you.
