Hello
Does anyone have any experience of completing this form themselves instead of involving a solicitor?
Any advice much appreciated

They are very simple. You can complete on line. The important thing is to read the information they give you. I have done 5 now and they really have simplified things since I completed the first one 7 years ago. If in doubt you can phone the Public Guardian Office. They are really helpful. One tip is to send the forms back by “signed for” delivery. They are not the speediest and you can track when it was received.

Also did them without solicitor. It was easiest for us to download and complete. I agree about recorded delivery

Yes we did it online. Very straightforward and couldn't believe how much the solicitor was quoting to do it for us.

We completed the forms while dad was in hospital, was straightforward.

We did it ourselves. Fairly straightforward as long as you are methodical and read all the guidance
