Self employed maternity payments..help!

I'm self employed and due a baby in 6 weeks. My midwife has signed a Mat B1 form, but I have no idea where to send it? Do I alert anyone that I'm taking maternity leave from when the baby is born? Can I work alongside getting the maternity allowance?

Google Maternity Allowance, you can get the application form online. You can do 10 keeping in touch days of work, but other than that you can't work while receiving MA. As long as you qualify for it, you can take up to 39 weeks.
