PPI Claim questionnaire

I have applied for a refund of PPI payments and the bank has sent a huge five- page questionnaire. They have the account number so I don't see the need for all these complicated, nosy questions. To be honest is was such a long time ago I really can't remember and I worry that I will disqualify myself if I give a wrong answer.

Am I obliged to fill this in? Any advice as to how to proceed?

There's no automatic "right" to the money back unless it was mis-sold to you initially. The bank will have asked you to complete this to check if you were mis-sold the policy or not. You are under no obligation to complete it, but you won't be refunded if you don't.
