P45 where do i send it? is it to late?

I have been out of the country for over 6 months now. I finished work in September and have a P45. I am not coming back into the country again due to illness.

I have been told by my HR department that I would be due a tax refund. Where do i send the form? Do i send anything with it? is there a cut off date to send it in by?

When I have claimed tax back in the past (student days) I always waited until after the end of the financial year and then wrote to the local tax office with my NI number saying that I believe I had overpayed and would they look in to it.
Usually got a cheque within a month.
I should imagine there are forms online for doing the same.
I don't think it can be done until the end of the financial year so you are not too late.

i just want to post it in. I lived in north Herts and cant find an address of where to post it. I also need to know what I should include with the P45 form.
