Is this a miscarriage or a hematoma?

I'm 6.1 weeks today. Up until today everything has been great. I first tested at 4.3 weeks and got 2-3 weeks on a digital test. From the very start I had sore boobs, nausea, food aversion, mood wings, and felt tired a lot. Then today I was constipated for the first time in months. I put this down to eating lots of bread and cake yesterday after avoiding wheat for the last few weeks because it blocks me up. Then after breakfast I started to feel a bit weird and noticed that I had mild cramps in my inner thighs, the way I only ever do during my period. Then that switched to pain in most of my left leg and the left side of my lower back ached. It feels kind of muscular though so it might be from yoga this morning. Then I felt a gush and went to the loo - it was red blood. And then I was in quite a bit of pain briefly. Then after that I just spotted a bit, no cramps, legs fine, but side still ached. Then side stopped hurting but I was bleeding a little bit and when I took my dog out quickly I felt pulling/tenderness in my uterus. Oh and when I did a digi test today it was only 1-2 weeks, but this was with super diluted urine, whereas the first was with very concentrated, several hours old urine.

My local epu is apparently only for pregnancies that are seven or more weeks.

Does this sound like an mc? Could it be a hematoma? What shall I do?

I'm sorry you're going through this.

I had a very large SCH, identified at the 6 week scan (IVF). I felt no pain at all, just several gushes of bright red blood. Unfortunately I ended up having a miscarriage anyway which was much more crampy and uncomfortable but I was around 9-10 weeks at that point.

I had some light bleeding in another pregnancy at around 5 weeks which was alongside some period-like symptoms. No hematoma visible on the 6 week scan this time. It only lasted a couple of days and the rest of the pregnancy went smoothly.

I think it's probably a case of wait and see what happens over the next couple of days.

I wish you all the best 💕

Thanks lots for replying @StepOff@StepOff.
Sorry you had to go through an mc.

Update: looks like I've stopped bleeding. There was no blood in the night or this morning. But I don't feel the same. I'm less nauseous and my boobs feel a bit deflated. Just feel exhausted and want to hermit on the sofa instead of enjoying the sunshine.

Update: I didn't bleed at all again last night or today nor did I have any pain and this morning my symptoms returned, along with a new food craving. I had a lovely gentle day in nature. But then when I got home I wiped and there was quite a lot of red, and when I wiped a few mins later there was a clot. So I guess that's that.

I’m sorry to read that 😢. It’s devastating when you think you’ve got past the CP window, have tests showing HCG going up, and then crash…take another test in a week and see what that looks like, but the CB going down to 1-2 weeks (so less than 153 HCG) at 6 weeks, coupled with all the bleeding is pretty conclusive. Take care xxx

Update: Didn't get any pain yesterday and no blood or pain in the night or this morning.

Have you taken another test

curliegirlie · 26/08/2025 08:03

And it might be worth booking an appt at the EPU. How much blood have you had in total?

MacRose · 26/08/2025 08:32

Haven't done another test. Don't wanna drive myself crazy trying to interpret it. Too many variables. I'm waiting for either a scan or to cramp and bleed enough to know it's a mc.

Not sure how much blood I've passed all together. Apart from initial gush and yesterday's fingernail size clot, it's mostly been when I while - definitely not soaking pads or anything.

Our epu here is for pregnancies that are at least seven weeks, so bracing myself for a fight to get referred.

It’s surprising they say it’s 7 weeks - most say 6. Can you self refer or does it need to be through your GP? If through your GP, it might be worth trying to get an appointment now so you can get one at the EPU as soon as you hit 7 weeks.

MacRose · 26/08/2025 14:09

So I caved and got another digital test. It says 2-3. Obvs not fmu.

So now we have:
On week 4.3 I got 2-3 not with fmu but it was several hours old
On week 6.1 I got 1-2 not fmu
On week 6.4 I got 2-3 not fmu

I've been spotting today, but not bleeding.

What a roller coaster 🥴

If everything carries on being this confusing I'll ask for an epu referral (we can only access it through referrals here).

Given you’re still spotting I think a referral to the EPU would still be a good scheme. CB 2-3 week means an HCG level less than 2,750. I got my 3+ on 26DPO/5w6d, which was about right given an assumed doubling of HCG every 48 hours from when I got my first BFP at 10dpo….so again it suggests it might need checking out if your HCG is rising a bit slowly. At 6w4d it shouldn’t make much difference if you’re using FMU or not. I’m not saying things definitely have gone wrong - the fact that you’ve got a higher reading than last time is positive- but it makes sense to not have too high expectations. I’ve got everything crossed for you - it’s such a horrible limbo when you don’t know what’s happening.

Just updating this for anyone who finds this thread when going through similar. I bled through the night and through the day today. Been a bit crampy but not awful, actually less painful than my periods tend to be. Digital test went back to 1 - 2 today (at 6.5 weeks). Symptoms have gone apart from slightly tender boobs still. It feels like a very straightforward mc so I'm monitoring it at home. Will keep doing tests till it says not pregnant.

I'm so sorry, OP. I know how torturous it is to go through this and monitor every symptom and feeling all day every day and the fear of going to the toilet because of what you may find.

I hope you are as ok as you can be 🤍

MacRose · 05/09/2025 14:38

Got my "not pregnant" today, finally. Hormones were strong for this one.
