PUL/Ectopic/Methotrexate/Huge Anxiety

Hi

I am just riddled with anxiety at the moment.

For context.

-Positive Pregnancy Test - 24th September.
-Thought I was having a miscarriage about a week later, lower back pain, strong period pains and passed a 'lump/blob' which was purple/grey in colour which i assumed to be the pregnancy. I can only describe it as a blob which looked a bit 'seal-like' with two parts either side that looked like what would be arms.
-Told I had a PUL at early private scan (7th October)

Pregnancy HCG results to date:

-435
-428
-535
-461 -METHOTREXATE INJECTION
-365 (admitted to hospital for dizziness so they did bloods)
-338 (Day 4 post injection)
-229 (Day 7 post injection)
I am now day 8 post methotrexate.

Whilst i am incredible grateful and relieved my bloods have dropped since the injection and surpassed the 15% target - I am so anxious regarding rupture.

I have this huge fear that I am slowly bleeding out and I just don't know - as I only have slight pains in my right here and there which if i wasn't going through this i wouldn't even think twice about. Would i know if i was internally bleeding? i just fear as i haven't had a scan now in over a week i am just living not knowing and my HCG has gone down because its bleeding out?

If my HCG is dropping does this mean my ectopic is getting smaller so I am getting less and less at risk of rupture?

I am also just so confused by the concept that three scans just couldn't see anything anywhere - does this mean I potentially didn't have an eptopic and it could be retained tissue? or does it mean it was so small to begin with that it was never a huge risk of rupture? Also the 'thing' i passed i am so confused as to what that could've been if there's still sign of an ectopic.

This is my first pregnancy and i'm just overwhelmed by the past 2 and a half weeks so i apologise if this post is super hectic - running on barely any sleep and a lot of sickness and tiredness from the jab - please feel free to drop also your experiences on the injection or your stories!

I’m so sorry for your loss and that you’re going through this.

I have also had an ectopic, although it played out in a very different way. I had no idea, it ruptured, I was rushed to hospital.

I can’t say what risk you might be at, but is another private scan an option (if EPU won’t scan you again)? It must be very worrying not knowing and — at least from my experience — they’ll be able to identify bleeding from a scan.

Bleeding into your abdominal cavity is usually very painful. If you aren’t having much pain, then you aren’t have much bleeding (if any).

I had an ectopic earlier this year too and im so sorry for your loss, the worrying is exhausting and its a rough time. Im not sure on the process with methotrexate but a scan will show internal bleeding. I had a scan at around 7 weeks my bloods were only around 80hcg and very slow rising, but showed internal bleeding and pregnancy in right tube so had to have surgery. Everyone is different but at that point I didn't have much tummy pain just tenderness, dizziness and shoulder tip pain. I also passed small clots and grayish (what I assume) was pregnancy tissue. I would see if the EPU will scan you again. Also ectopic pregnancy trust is great for information and advice so may be able to help x
