Pregnancy with unknown location

Hi has anybody had any experience with this.

I went to the EPU today as I’d had some spotting (6 weeks), had an internal scan and they couldn’t see a pregnancy but could see some blood within my uterus.

as I’ve only had very light bleeding they don’t think I’ve passed the pregnancy so have said that it has either collapsed or is ectopic. At this point I’m hoping it’s not the later but I don’t have any cramps or pain and the bleeding has stopped.

can anybody off advice on how this situation panned out for you? I’m so worried it’s ectopic!

Hi @Grace1992 so sorry you are going through this. I have just had an ectopic loss. At 6/7 weeks EPU confirmed ectopic in my right tube and I had internal bleeding showing on scan. I was spotting at first but the bleeding got heavier and consistent so I thought I was miscarrying. I had a heavy ache down my right side, shoulder tip pain and felt dizzy on the day they confirmed ectopic. My hcg levels were very slow rising. Have they been monitoring your hcg levels? X

Thank you so much for replying @pinkrose001 feeling very alone in all of this and don’t know anybody that’s been in this situation. So sorry to hear you’ve had to go through this 😢

they took my bloods today and said I’ll have to get them checked every 48 hours. But my results hadn’t come back from todays so they said I’ll get the result from those in the morning.

They seem baffled as I don’t have any pain that they’d expect with a miscarriage or an ectopic so it’s a waiting game. They couldn’t see my tubes on the scan they said!

@Grace1992 didn't want to read and run as know exactly how lonely and worrying it all is ❤️ I never even knew what a pregnancy of unknown location was until I went through it. Looks like they are monitoring you closely and hcg results will be a good indicator to what could be happening. The first scan I had they couldnt see anything either so monitored my bloods then had a second scan 2 weeks later x

Thank you @pinkrose001 do you mind me asking how they dealt with the internal bleeding, could they save your tube? I hope you don’t mind me asking. I’m just so worried I’m going to end up needing surgery.

Sadly because I had internal bleeding I was told I couldn't have the injection and had to have surgery which resulted in the loss of my right tube. They found it had partially ruptured in surgery too hence the bleeding. They did say this wouldn't affect fertility too much as your tube moves each month to collect the egg from either ovary (I'm not sure on the actual statistics re fertility) and have been reading lots of women go on to have successful pregnancies so trying to hold on to this for hope x

@Grace1992 i am currently waiting to be discharged after a PUL ended up being ectopic. It took 3 scans over a week and a bit to find it but they did and i got methotrexate
I got admitted as the pain got bad and there was blood on the scan but they think my tube has managed to push it out the end and I can go home.

It is so scary to not know, i had come to terms with it being another miscarriage (i had a mmc in jan) that i was thrown by this news but if youre under the care of the EPU you're in the right place.

So i dont have a final conclusion yet but i had internal bleeding with an ectopic and they aren't removing my tube (so far!)

@pinkrose001 im so glad you’re ok now and it sounds really positive that it won’t have much of an impact.

@FullMoon1917 im so sorry you’ve had to go through this to. I really thought after my missed miscarriage in May that I surely couldn’t have more bad luck and here I am too. It’s cruel that it happens. I’ve got everything crossed for you that you don’t have to have any further treatment.

im back tomorrow morning for another blood test so will hopefully get an idea soon if it is ectopic

I just wish I knew the reason why for the 2 losses I’ve had. I’m so lucky in that I have a little girl who I had no problems with

To give you another perspective, I had a suspected ectopic and my scan yesterday showed not ectopic, but still no embryo so a missed miscarriage.

It's still devastating but not with the additional worry of ectopic. I'm so sorry you're going through this

Yes . I ended up doing ‘expectant management’ having bloods taken every 2 days (I think).

in the end after about a week of back and forth to the hospital I spoke to a fairly angry doctor who was basically like ‘what are you doing? If we don’t do something about this now, you could end up in a very bad way’. So I had methotrexate. Really didn’t want to as the ectopic was only suspected and not confirmed I.e couldn’t actually see it in the tube. But had a healthy pregnancy the one after. Good luck, it really isn’t nice to experience x

@carnation2531 thinking of you it really is the most horrendous time I wouldn’t wish it on anybody.

@Pinkelephant66 thanks for replying, can I ask was the injection ok, I’m worried about side effects if that’s the route I have to go! It’s great to hear you had a successful pregnancy afterwards, after 2 in a row I’ve just about lost all hope.

@Grace1992 the injection was fine. Luckily I had no side effects at all but I know some people can experience sickness with it. But it’s probably better than potentially having to have surgery to remove a tube.

I wish for it to be third time lucky for you!!

Hi OP, I always respond to these threads, as, like pp I'd never heard of a PUL until I had one and it was such a lonely, confusing, and worrying time with the ectopic question hanging over me. In my case it turned out to just be collapsed in the uterus ie not ectopic. I had a d& c and a successful pregnancy shortly afterwards. Wishing you the best of luck- you're not alone x

@Newnamesameoldlurker thank you for replying I really appreciate everyone’s support it’s much better than feeling alone in this. I’ve got everything crossed that mine is a collapsed pregnancy. Could I ask how long it started for you to miscarry properly as I have been spotting since Monday but it isn’t getting any heavier, if anything lighter. I got my blood results back from today and my HCG had risen 😩 only by 20 something so not enough for it to be an early pregnancy so they are now leaning more toward ectopic and I’m just hoping something changes.

@Newnamesameoldlurker I meant to say as well on my scan there was a 4mm pool of blood in my uterus so I’m hoping that is the collapsed pregnancy but then my hormone levels going up have thrown me off. It’s been nearly a week now and I just want it to be over 😭

@Grace1992 how are things now? My hcg has dropped from 2000 to 1000 so it looks like the methotrexate worked.

I will say for me methotrexate was really tough but I think it triggered my ibs and having intense trapped wind pain for days was not nice. But i think its very dependent on the person as to if there were side effects.

You poor thing OP. I didn't start to miscarry on my own- they monitored my HCG for a week and it was rising but not enough so I opted just to have a d&c to get the whole thing over with as I couldn't stand the limbo. Really hope this is all resolved for you soon - it's such a massive headfuck
