Eptopic advice?

Not sure how far along I am I think I am 4-5weeks (pregnant after miscarriage no period) hcg only 50% increase in 48h from 2000 to 3000. Scanned today and they found nothing. They said there may be a mass next to my right ovary. They aren't sure. They gave me a leaflet for eptopic pregnancy. Said theyl do bloods for the injection. She also said something about tube removal but don't see how this is possible since they aren't sure on what they saw. This is my 2nd loss now. No children. Any advice? Thanks x

Im really sorry you’ve had a PUL at your scan today.
I also had the same 4 weeks ago.

your hcg is expected to rise 50% every 48 hours at this stage, and yours is doing this.
this on its own isn’t a concern obviously it’s the findings from your scan.

they have to give you all the possible options when they can’t see a uterine pregnancy, one of which is ectopic.

Some ectopics resolve on their own and you will miscarry naturally.
some people choose to have methotrexate (which I’ve had) and for some people they go down the surgical route. They remove the whole tube to be sure they have the entire ectopic but also because if you have another working tube it’s better to take this potentially scarred/damaged one to lower the chances of a further ectopic in this tube.

at this point I’d be asking for another scan alongside your bloods - which tbh they’ll prob offer anyway as they’ll want to be sure about the possible mass.
from having 4 scans, my advice now would be to ask for a consultant to complete your next scan as for me, they have been far superior in terms of thoroughness and advice/options during the appointment.

im sure they said but while you are waiting for your next lot of bloods etc, make sure you return to hopsital if you have an severe pain (usually one sided) or shoulder tip pain.
not wishing to scare you but there always the chance of rupture with ectopic so be alert to this and go to a&e immediately if you feel something isn’t right.
not all pain is rupture, I’ve had 2 lots of bleeding and severe one sided pain during my experience of this and it’s been me passing pregnancy tissue etc
I also packed myself a little hospital bag just so I was prepared to leave the house in a rush if I ever need it.

Thank you. Did the injection work for you? They called me and said they can offer the injection today or tomorrow. They done 3scans and still can't find anything

Hi,

my hormone is currently on 45 and I’m back next week to hopefully confirm
I'm at or there abouts - but so far yes it seems to have worked.
Although my case has been quite complicated, I naturally misscarried a lot of tissue on the 23rd July and my hormone went down but only to 50 then rose again.
the methotrexate seemed to take a little bit longer than usual to kick in with me.
Once taken (I was 151 hcg) you return for day 4 and day 7 blood tests. Day 4 it’s usual for it to go up a little as it doesn’t work instantaneously -mine did rise to 221 Which is over 50% which wasn’t ideal. I returned for day 7 and it had only reduced by 3%. They were looking for a 15% reduction from day 4 at least. I was offered another injection or surgery. I went home to think about it and when I returned 48 hours later it had gone down 30%. I searched through peoples MTXs experiences online and only found one other person who had had similar to me and had waited till day 10 and got the big drop needed.

I had been pretty much symptomless the entire time apart from being a little tired.
however on day 14 I got severe pain and started to bleed, hormone dropped to 45.

other peoples seems to have been more straight forward with only a little rise (or even a small decrease) on day 4 and a nice drop by day 7. So don’t let my slightly unusual experience put you off. Some
people will need a second injection so that is also a possibility.

Every one is different, some people have a lot of bleeding and discomfort, and a few feel fine. The injection itself is pretty painless, injection site twinged for abit but nothing major at all.
downsides to the injection are no TTC for 3 months (6 if you need 2 injections), no drinking to help your liver recover for the first few weeks at least and immune system may be low so avoid large groups for the first few weeks too.

Good thing with the MTX is it works well for most and will likely give you a much quicker resolution that waiting naturally - alongside the fear of rupture.
