Has anyone changed their child's surname

Hi
I want to change my little girls surname, her dad has agreed. There's loads of websites. I went on gov.uk and it seems really complicated. Do I need to get a solicitor to sign as witness on the affidavit? Does my ex need to be there too? I thought it was as simple as filling out a form, signing it and sending it back

You can do it by deed poll. But beware, my ex urged me to change our daughters name and wrote a letter stating all the reasons why she should have my name rather than his. Now we are court proceedings and this has been used against me, with him citing parental alienation, that I did it without permission, that I threatened him. Thankfully I have his handwritten letter.
