Travel insurance for expats

We travel a lot from our base in the ME. For the last couple of years we have had our travel insurance through Columbus direct. However they do automatic renewal, so we got caught out last year. So this year they increased the cost by 10% and the previous year by 17%. Given we have NEVER made a claim (touch wood) it seems a tad steep. Columbus say they can't do anything and won't put me through to a manager or customer retention.
So alternative suggestions please. Thanks.

We got ours with our bank account in the country we were living in, hsbc.

Same, Hsbc or with the Amex.
Check your credit card, some offer automatic cover when you purchase the trip with the card.

We do have HSBC premier. So I guess if I check we will have travel insurance. So I suppose I need to read their medical package. That's of most concern to me.
