Palliative care letter

My Dm has pancreatic cancer and has a couple of months left at most and she has decided she only wants to deal with doctors and palliative care team at the hospice. Her doctor has said she needs to put this in writing as soon as possible while she still can. I’ve been asked to do this for her but I have no idea what to write. I’ve tried mr google but got nothing back. Does anyone what to write please?

Would the hospice be able to give you any advice on wording? Or Macmillan?

So sorry you find yourselves in this position. My dad doesn’t have long either so I’m in a similar boat.

Do I just call Macmillan or have to make an appointment to meet with someone? My DDad asked me to do it for him and I said yes but now I’m stuck what I actually need to do.

I’m afraid I don’t know for sure. But I would imagine if you call the hospice they would offer some help as they must’ve seen this before.

Bless you, I’m sure you’re desperate to help in some way, even if it’s writing this letter. I like to feel useful too so jump on any job I’m given.

Life is indeed a funny old game.....

She needs to make an 'Advance Decision' (also called 'living will') if you're in England or wales, known as an Advanced Directive in Scotland. This will be a formal, legally binding document to communicate her wishes.
