Passport name change

I changed LO’s name via unenrolled deed poll, me and DDs father signed it and got 2 witnesses.

applying for new passport for her with new name,‘it’s asking for a supporting statement from us both to confirm we approve the name change.

will the deed poll document with signatures and witnesses not suffice? Or do I actually need to type out a letter and get him to sign?

You don’t need to type it and get him to sign, no. You just both write one each. We did this when we changed DS nam last year. Identical letters signed and dated by each of us. It took all of 5 minutes. I would say that was by far the easier part of the process.

Just get him to write a dated note saying ‘I am first name surname, I am baby’s name’s father and I agree to the new name of : x’ and sign it
