Is it worth doing FDA when no questionnaire and no form as received - complicated by non mol and no solicitor as contact point - help

I am admittedly late on my form E submission as I have health issues. However it’s less than 2 weeks to the FDA and I haven’t received his form E as he can’t send it to me directly. ( I got a NMO against him). He is now litigating in person. Court not replying to me re his form E. What shall I do now? Send my form E and papers in without having time to do the questionnaire? Or adjourn and ask for proper directions of dates? Sorry but I am exhausted with health issues. I also worry he had debts and if will be liable and best to just ask for hearing remotely. It’s 80 miles away as he moved so I can’t do in person anyway. Thank you
