Does anyone know anything about these, please?

I’ve spent ages filling one in and didn’t realise there’s nothing on the site or form telling me where to send it! I’ve emailed the court but got an automated response telling me they’re experiencing delays etc.

You send it to the court.

The guidance notes refer at page 2 to sending it to the court.

Ah sorry - I’ve been in a bit of a panic, so just filled the form in without reading the guidance notes!
