Deed poll to change a child's name

I'm looking to add my surname as an additional middle name for each of my kids. Have an amicable separation with their father, and want them to have both of our surnames for convenience when travelling, both parents in agreement. I saw another post from 2017 on here stating that it's not actually necessary to officially send off the deed poll forms to the High Court (unless court orders in place etc), and this is just a formality that is not used for most people. There were many replies under the thread echoing this, and saying that they just filled in and printed off the deed form and took that with them to places where they needed the name change applied.
So really, I would just love some clarification, as I need to update the kids' passports soon. Do I just print off the LOC022 form and include it alongside the passport application? Would I need to have anything separate in writing from both parents declaring that we both agree to the name change, or is the form enough by itself?
