Dismissed 12 months ago because of disability, just found out employer did not consider reasonable adjustments. Is there anything I can do?

As the title says really. I became unable to do my job because of a disability I developed. I asked at the time for help to get back to work but this was refused. Was dismissed 12 months ago on capability grounds.
I applied for an ill-health pension. This was refused. I appealed the decision in September 2022. My employer refused to do anything with my appeal (per regs had 3 months to deal with it). I eventually received the outcome in December this year (15 months later) refusing my pension as workplace adjustments would allow me to return to work.
I am obviously well outside the time limit for a discrimination/ unfair dismissal tribunal.
Is there anything I can do?

My professional advice - speak to ACAS or go straight to an employment law professional.

Consult a solicitor who specialises in employment law. Check if you have legal cover through your home insurance. They may be able to help if you do. You may be able to make a claim out of time, but you need to move as quickly as possible.

Not in a union (membership would have lapsed anyway).
Will call ACAS and check insurance.
There is a lot more that employer did that I can evidence so hopefully I will get somewhere.
