Changing DD's name help please

Hi,

I'm wanting to change DD's surname- still keeping her dad's name but double barrel to include mine. He won't agree it so understand I need to go through court but I can't find what I actually need to apply for. The form the gov website takes you too had nothing on there where you would request for name change it's all about parental rights/visitation. Not sure if I'm just not reading it correctly.
If anyone can help I would appreciate it.

I’m not sure you’re actually allowed to do this without the father’s permission until your child is 16 and deemed old enough to make the decision for herself.

Ah okay, looking on the gov website it words it like if the other party won't agree you can take it to court for them to decide but I could be reading that wrong x

Are you doubling with your maiden name or your new married name? My mum double barrelled my name with her new husband’s and I HATED it. I know DB is more common nowadays but just bear that in mind.

With my maiden name with the idea if I ever marry again I would just double barrel my married name (can't see it happening though haha)
