My parents split up so I cash her my surname to my mums. This was 2004. I know it was done by a solicitor. I don't no where my Paper work is. I need a copy as I need a dbs to work in a school. Does anyone how I could get a copy?

Was it done by Deed Poll?

My mum took me in to the solicitors office so I'm not sure. I didn't need my dad's permission so must have been 18 I would guess

There's a process to register deed polls with the courts but I'm not sure how long that's been the case and as of now it seems to be optional:

My suggestion would be to try them first. If not do you or your Mum remember the name of the firm you did it with or where their offices were?

More info here:

My first thought on finding the URL etc was that they were like those outfits that charge you a fee over and above what you need to - like for a passport - but in fact there's a reasonable amount of free info there.

Just get a new one done. There are plenty of free templates available online. No need to enroll it with the court. Just make sure you have lots of copies/originals.
