My ex is taking me to court and I need to fill this out does this mean are the children been at risk of being abused from their dad?

They are asking you if you believe the children are at risk of any of those things from ANYONE who's involved in their care or who lives with them or has contact with them.
So that could mean their dad, or someone he lives with or hangs out with or is related to, for instance. Any fears that you have in this area.

Ok well their dad doesn't have contact with them yet, he's trying to have contact with them. So it will be a no then? they live with me and my partner and other siblings and they are not at risk of suffering any form of abuse from us

Well it says has or has been involved with their care, so presumably that includes your ex? Did you live with him and the children? Does he have parental rights as things stand now? Then the answer is yes. They are trying to find out whether he should be excluded from their care.
Have you got proper legal advice OP?

Yes we lived with him that was over 2 years ago and yep he's got parental rights. I'm waiting for a solicitor to get in touch with me, I've contacted a few.

So has he ever abused you or the children financially, physically , emotionally or mentally or neglect them - ?if you write

mr John smith (applicant) physically abused me on 01.01.22 at 8.00 am in front of the children Mary smith and Peter smith - reported to the police. Injuries were : -…,., I attended hospital on 01.01.22 at 16.00 and had an x ray and ….. or whatever they are

He's physically abused me but not Infront of the children he's verbally abused me over messages and he's very aggressive and violent so I do believe if anything happens whens he's got my children he will kick off and I don't want the children to witness anything like that.

Reported everything to police that was over 2 years ago now even this he's still harrasing me the police are still involved
