Help filling D11 please

Hi,

I need help with filling the D11 please.

Q4 - any reason application should not be dealt with on paper by a judge? I ticked NO

Q5 - any reason application should not be dealt with at telephone hearing? I'm not really sure about this. I'm going to represent myself, maybe have a mackenzie friend for support so I rather not have a telephone hearing. Obviously the court is not going to ring me at home for hearing?

Q10 What information will you be relying on?
Do I need to attach my decre absolute or the consent order is enough.? The consent order has been sealed and has my divorce reference number.

Without context it’s impossible to answer, but a message board isn’t the place for asking these particular questions.
