Leaving a property

Hello just looking for some advice really.
I live by myself in an apartment. I moved in July but ended up recently having a mental breakdown, like in the hospital getting intensive support and it's not safe for me to live in my apartment as I am by myself, I will be moving in with my dad.
Does anyone know if I have any rights to leave early due to my health or what to do as I can't work right now as I am with an intensive support team for 2 hours a day.

Your support team should be helping with this. Have you mentioned it to them? Have they asked?

Depends on your contract but in reality you will have a minimum 6 month tenancy with a two month notice period
