I have been asked to complete one for work.

Where might I get one and is there a standard fee?

An affidavit is a sworn statement. If you're doing one for work you really ought to be getting them to set all this out for you. If it's for court proceedings there is a standard form here: www.gov.uk/government/publications/form-n285-general-form-of-affidavit

Thanks collaborate - I have the form it's just working out who can verify it for me and I've discovered that it looks like any solicitor can so that's my next step.

The solicitor doesn't verify it. You swear on oath before the solicitor. There is a fee payable of £5 plus £2 for each exhibit.

People regularly come to me with a document to swear then hand me an empty form and expect me to fill it in for them. They tend to get it straight back. I presume that's not what you mean by verify the form.

Thanks. I've completed the form, spoken to the solicitor on the phone - he kindly said he'd print it for me as I've just moved and am without one - and is charging £7 for my/his sweary bit.
