The witness has put their signature in the name bit and vice versa. I'm presuming the deed poll is useless now?

Mine only had one witness and was on normal paper.

Yes, banks, DVLA, passport, tax credits, benefits, various nhs jobs etc, had an issue with hsbc I think, but just closed my account and carried on with Santander.
