Free name change for a child?

Apologies if this has been done to death on here but I couldn’t find anything recent.

We want to change DSs surname to add mine to it along with DPs… DS is 10.

I was hoping to do this for free but can’t find any of the free online templates for changing a child’s name – they all seem to be for adult’s changing their own name.

Can anyone direct me to where I could do this myself, or will I need to pay someone????

Sorry to be tedious, but as you're posting on Legal, have you got the consent of the father (assuming he has parental responsibility)?

Hi, yes, DP is the father! I should've made that clear!

DS currently already has his dad (DP's) surname but we want to add mine. (Should have just done it when we registered birth but thought I didn't care. Turns out i do!)

I posted in legal as I thought name change sounds like a legal issue but perhaps it would be more appropriate elsewhere?

That’s the site I had seen, but it seems to be only for if I were changing my own name... unless I’m missing something?
