D50k form - consent order enforcement

Hi all! I’m about to send a D50k form to court to enforce a financial consent order that my ex is not following. Does anyone know how much it cost? No one seems to know and the sites online are not clear. I called the court to find out and they told me to call Citizens Advice Bureau!? Tks

Another question on the D50k... the consent order was made in Bury St Edmunds divorce court. Do I need to send the D50k there? I can’t travel there. I was told I could apply to my nearest family court?
And do I request for an attachment of earnings at the same time with the same form? There’s a non-molestation order against my ex in place, so we cannot negotiate ourselves, not even via mediation. I know he can afford. Tks

They will issue it to be heard in your local court, but no harm anyway in reminding them.

Thank you! I was told I could send the form to my nearest court. Sorry, but I have given a lot of incorrect information in the past so now I’m paranoid.
And could I say in my statement that I think the best way to recover the sum is via an attachment of earnings? Or should I wait until the court assess the form first and take it from there? My ex has already said he will not pay even if the court orders it. There has been other court hearings on other issues and he has not attended any. Tks
