Specific Issue Order

Few questions if people don't mind:

Do I need to apply for one every time I want to take my daughter on holiday (if her dad won't give consent)?

Do I need to have booked the holiday and have exact dates before applying? All I know at the minute is it will be two weeks in August 2019. I'm nervous to book anything in case the court process is long and drawn out. Plus I might not have all the spare cash to book it until closer to the time.

Would a child arrangements order be better? We currently agree (for the most part) on contact dates but he's said he won't give consent for holidays. I've had to change my phone number today because when he gets pissy he's aggressive (not in person, I don't have to see him) towards me. Textbook narcissistic.

I don't want to waste a courts time, I'd feel more comfortable having a CAO as at least I'd have something that says she lives with me. He's always threatening to go for custody, he wouldn't get it, but he knows the threat is enough to hurt me.
