Guardianship for person with disabilities

I am hoping someone can point me in the right direction and also give me some advice on whether I need a solicitor or if I can sort this out myself. My ds has severe asd and learning disabilities. I am his appointee for pip. He is nearly 17 but will soon be moving into adult services. I would like to maintain legal responsibility to make decisions for him so I can be sure he is placed in the right kind of provision. What do I need to do to ensure that I can continue to make decisions about his future care?
