Im self representing and have the first hearing on Friday, do I need to fill out a form h as it says on the court letter? I have never had a solicitor and not incurred any costs??

Fill it out (you may have paid a court fee).

I havent got the form but on looking at it online theres nothing on there to say im self representing or have no costs to declare..

If that's the case no one's going to be worried if you don't file one.
