help! how do I apply to extend spousal maintenance

Could anyone advise me which deptment of justice form I need to complete to apply to extend spousal maintinence that is due to end soon, but will leave my child & I in hardship if it does?
Any help much appreciated, I cannot afford any legal help. the exsisting order expires in a couple of months so need to apply asap. Thank you

Is it reasonable to extend. How old is your child OP and how long have you had SM up until now? Is there a reason you cannot work to support yourself and your child with wages and child maintenance?
