It's a written declaration of facts, signed by you in the presence of a solicitor or similar, much like an oath or affidavit.

What do you need it for?

Instruct a solicitor. Probably be anything between £60-£300.

Proof of residency offer a certain time that I do not have proof for (it has been lost when everything switched to computers)
