5 years separation - wording

Hi
Can anyone advise what wording I need for part 6 of divorce papers for divorce on the grounds of 5 yrs separation? From reading around, I understood I just needed to state when one party moved out (i.e. over 5 years ago) but ive had the papers sent back to me. Must I still give examples of unreasonable behaviour? I thought that was just for under 2 years separation.

No need for any behaviour allegations. You just need to set out the circs, e.g."the parties' relationship was in difficulties throughout 2010. On or about 1st November 2010 they agreed that the marriage was not going to succeed and the respondent left the former matrimonial home on 15 Nov 2010. The parties have not lived together since thar date"
