Step-parent Parental Responsibility Agreement Form

My wife and I are looking at getting a form signed, so that I too will have parental responsibility for my step-daughter.
Our case should be pretty straightforward, as the child's real father has never been involved in her life, was never married to the mother and because she was born before 2003, he does not have parental responsibility anyway.
So we only need my wife and myself to sign the form.

My question is for the other parent part of the form it says to delete if there is only one parent with parental responsibility.
So should we edit the form electronically to delete that section, or print out the form as is, and just cross it out?

Also my wife's ID (passport) is still in her maiden name, as she's not changed her passport to the married name, so would it be better to enter her name on the form as such, or will her ID and a marriage certificate be good enough when we present the form to the court to be witnessed?
