specific issue order under new legislation

Can anyone talk me through the process? Summer is coming and it's holiday time so am failing yet again to get letter from ex for travel abroad. We have contact order in place under old system but have had to go to court for permission to travel on several occasions (it's not me, it's him and I've had permission granted each time)
Mediation tried and failed a very long time ago and wasn't asked to attempt it last time but that was at the end of the old system.
Is it also possible to ask for judge (s) who have had the pleasure of hearing the same old stuff to hear this application? I'm getting very bored of the process and it's an additional expense even though I self-rep - the cost of the application and I don't want to add cost of failed mediation).
Is it possible to ask court to give permission for holidays abroad in general or does it have to be for each specific trip. Or do I have to make application for whatever the equivalent of residence is now called?

Don't know anything about new system, but DH applied to amend an existing contact order to include holidays and that states we can take DSS abroad every year - would that be an idea?

It's all very acrimonious and horrible. What I'd really appreciate is knowing what the court form is now called for a specific issue and does it still exist.
Also does the new legislation mean that you have to try mediation again? I don't see it working as it didn't before on two different occasions and I just end up paying more and then still end up in front of a judge
So more of a procedural question.

Specific issue still exists and it is still a C100. I hope someone more knowledgeable comes along soon to help - but I do think you'll have to do mediation again. Definitely ask them to vary the order to cover holidays abroad in future.
