Lost Drivers Licence

So I lost the counterpart to my drivers licence!

However I need to change my name and want to change the photo on it as well. However I want my passport photo on my drivers licence instead of the one i have now also!

I have the application and have filled it in and checked the Lost drivers licence, change name and change photo boxes.

1. Will they pull my passport photo automatically or do I have to provide it? because I don't have a copy.
2. Where do i send it to as I've lost the booklet!

You have to send the photo you want going onto it, they can't just take it from the passport.

Send forms to DVLA, Swansea, SA99 1DH

You can use your passport if it's under two years old and then they just take it directly from the passport.

If you have lost your licence, you just tick that box. I had
Lost my photo card and it was fine. I just had to return the paper part but my new licence was with me within a couple of days. Before I had posted my old one back.

I did mine online.

You have to be very careful that you go on the actual dvla website and not one that charges you for doing it.
