Commercial Freehold Lease?

My Father owns a commercial freehold. The address on the lease is his old address but he is now in a care home with dementia. I have Power of Attorney over his affairs.

Which address should go on the lease, his care home or my address?

write to the tenant and explain the situation with a copy of the POA and confirm your address as the one for service . the lease should have a clause that deals with service of notice. you may also need to sort out the rent too if there's a review coming up etc

Make sure you use recorded delivery when you write to the tenant.
