Not really sure where to put this here seems best fit .

Hopefully be applying for a mutual exchange soon and I'm wondering if it is common for them to be refused ? The woman we are (hopefully) swapping with is single in a 3 bed and will be moving to our two, so although to a smaller property she will still be underoccupied . Will this be refused ?

It could be refused. Many housing associations and in particular local authorities would not allow it, but since she is already downsizing from a 3 bed they may allow it.

There are no hard and fast rules and much will depend on the allocation procedures of the HA/LA she is moving from and to.

I thought the same . I think it's crazy that they would leave a single disabled woman in a three story house and family of 4 in a wee flat . I need someone to come tell me they were refused because of under-occupancy so I stop getting my hopes up . I need a reality checkGrin
