c100 form - what to include

I'm helping my brother fill out a c100 form to apply for access.

I'm just wondering if what we have to include with the form, is a parenting plan a good idea? any other things we could include?

No you just put what it is you are applying for and the brief reason why e.g. contact has broken down since a particular date and the application is for contact or shared residence. The opportunity for submitting a proposed schedule comes later.
