First Letrozole monitoring scan tomorrow

Hello, I'm on my first of 3 medicated cycles for secondary infertility. I've been advised against IVF due to v low AMH by my consultant. I ovulate on my own so this is a bit of a last ditch attempt to see if I can give my daughter a sibling (TTC 5 years between her and a second). I've been on 5mg Letrozole for a week and I've got my only scan of the cycle (to keep costs down as I've gone private) tomorrow. It's on cd11 which feels early but day 12 falls over the weekend so it's the latest they'll do. Can anyone with experience shed any light on whether they think this one scan will be enough? Aware this might be a silly question and that I possibly won't know anything until tomorrow but this is all new territory for me. I'll be having a trigger shot too and seem to ovulate anywhere between day 13 and 17. My OPKs are very light. Any other advice welcome too as feeling pretty exhausted by it all.

Hey @secondtimelucky87 ,

Hope your scan went okay? From my experience the one initial scan (CD12ish) has always been enough for the doctors to determine if the Letrozole is stimulating a dominant follicle (s) or not. On one occasion I was brought back in for a scan a couple of days later to check my body hadn't started responding later, but there was no change. Usually by CD 12 my follicles have been around 18mm so they can see there is a dominant one and know I will ovulate in the coming days (usually around 4 days later).

Aww thank you so much for your helpful reply. I did have 3 follicles on cd11 with the dominant one at 18mm and my lining was over 8mm. I'm now 5dpo so just waiting to see if it's worked. I really appreciate your message. Am I right in thinking the lining was a little thin or might it have got thicker within the next few days?

That's great news!! :) Glad you got the positive news you were after and 3 follicles is incredible!! I have only ever had one so shows your body responded positively to the medication.

My lining has always been around the 7-8mm mark on CD 12, so I'd say yours at 8mm was perfect. My consultant has always been happy with that, and like you said it continues to get thicker in the run up to ovulation and afterwards I think.

Exciting you're 5dpo.. although I find the TWW drags SO much more than the lead up to ovulation haha. Good luck for when you test and I hope you get the result you want x

Thanks so much. I'm a bit clueless about it all so it's helpful to have some insight from people who've been through it. That's good about the lining and the follicles! We have had a couple of poor semen analyses in the past but the last one -after a few changes - was perfect. So in theory, aside from my low AMH, there shouldn't be anything 'wrong'. I'm ovulating, clear tubes, have conceived before etc. But obviously so much of it is just down to pure luck. It's been so different not temping this cycle (didn't bother after trigger) as I normally know when roughly to expect AF. So it really just is a waiting game now. I'm definitely hormonal but that's normal PMS too so who knows!! Thanks again. I hope all is well on your journey too x

Yeah so tricky isn’t it.. like you say, it feels like such an exact science but people get pregnant without even planning it all the time!! I agree with you though, if the ovulation element is now solved then we’re in the same boat as everybody else who is trying so no reason for the letrozole not to work :) having said that, I have to remind myself that even on months where everything aligns perfectly it’s not a guarantee - which is also crazy!!! Bit of a waiting game but sounds like we both have good chances of it being successful. Good luck to you too! X

Hi there, sadly it didn't work for me and I had two failed rounds. I responded well to the letrozole, producing two or three good follicles. Good lining. Everything timed perfectly etc. I also have low AMH of 2.85 at last check so potentially influenced by that I suppose. I'm now taking a break while I try working with a fertility coach to see if there's anything I can optimize. It's frustrating when you're told it's unexplained secondary infertility as I'm sure there's a reason - just need to get to the bottom of what it is. I wish you so much luck with your treatment. I did find it so helpful monitoring what was happening and knowing we were trying at exactly the right time etc 😊
